Director, Clinical Operations & Care Management

Daelene oversees the Populytics Care Management department, focusing on strategy implementation, compliance, delivery of services, adherence to policies, and efficient use of resources. She is also accountable for process and quality improvement efforts to achieve and/or maintain compliance, collaborating with LVHN in population health and with BeneFIT Corporate Wellness to provide patient-centered programs and minimize health plan risk.

Daelene comes to Populytics with more than 30 years’ experience as a nurse and clinical leader in varied settings including health insurance, home care, skilled nursing facilities, adolescent alcohol and drug treatment facilities, and medical/surgical hospital nursing units. Her background encompasses call center experience, population health management, operations and program development, quality and clinical systems improvement, client engagement, and utilization management.
